What is the healthelife portal?

The Health e Life portal will include your health information from the hospital, its services and its Specialty Clinics. It's all now in one place. Hospital staff are available to help patients get signed in to the new HealtheLife portal and add all of their pertinent health information.

The Carbondale Family Health Center, staffed by physicians and certified physician assistants, is located in the Professional Building at 141 Salem Avenue, Carbondale, PA.

How do I contact MHSC’s medical records department?

If medical records are needed during this transition, please reach out to MHSC’s Medical Records department at 307-352-8420. For medication refills, scheduling requests or other general medical concerns, please reach out to your provider’s office directly. Create your new account now.
