To obtain a copy of a medical record from Memorial Healthcare System, you must complete and submit the Authorization for Release of Confidential Medical Records Form. This is a Memorial policy, in accordance with federal and state laws, to protect the privacy and confidentiality of our patients' personal medical record information.

Hours of operation are Monday through Friday from 7:30 am to 5:00 pm and Saturday and Sunday from 8:00 am to 12:00 pm (excluding holidays).
