All of the information in My Patient Portal comes from your Electronic Medical Record. This ensures that you have access to the most accurate, up-to-date information possible.

The enrollment link expires 90 days from the day they are issued. If you are not able to find your invitation or it expired, you may request a new invitation from the staff at the facility that assist with your medical records. If you need technical help to locate the invitation, assistance enrolling, or navigating within My Portal (Patient Portal), you may contact our Technical Support center at (844) 274-8497; the staff are available 24 hours a day, seven days a week.

Technical support staff can help locate the invitation, provide assistance enrolling, logging in and navigating within My Portal (Patient Portal); however support staff are not able to assist with clinical questions.

An EMR is a digital version of a patient’s paper chart that can be shared by participating doctors, hospitals, and patients. The Epic EMR is a computer system that stores your information in a secure database. Results and notes from a visit to the hospital, physician's office, or outpatient facility are consolidated in one place. That means if you move or are traveling, participating providers can quickly access your medical history, allergies, medications, and test results to deliver the best care possible.
It is used by more than 250 healthcare organizations nationwide, including major academic medical centers such as Johns Hopkins Medicine, Cedars-Sinai Health System, and Cleveland Clinic. To date, more than half of all patients in the U.S. have their medical records in an Epic system.
For caregivers or dependents, a parent, guardian, or family member can be given access to a patient’s MyChart account. If you are a current MHUP patient, have visited the Memorial emergency department recently, or were a recent inpatient at Memorial Health University Medical Center, register for free.
Yes. Your physician can show you test results and medication records while discussing your care. You can view your information at home by enrolling in MyChart.

If your wishes change, you can complete a new form and provide it to Springfield Clinic. Only the most recent directive will be used.
Remember, Springfield Clinic offers ALL adult patients a chance to complete an advance directive. It doesn’t depend on your current health, and it doesn’t mean that your doctors expect your health to worsen. If you choose to complete an advance directive, Springfield Clinic will store a copy in your electronic health record (EHR). If needed, it can be used in the future—in one year or thirty years from now.
