To enroll in the patient portal, please call the clinic at 503-249-8787 to provide your email address so that we can send you an enrollment invitation.
The enrollment invitation will be sent from patientservices@medfusion.net. If you don’t see the email, please make sure you check your Spam/Junk folders. Within the email there will be a unique link that will take you to the Patient Portal where you will need to confirm your identity by providing your Birthdate and Zip Code.
The first time you login to the patient portal it will generate a medical records request to your electronic chart covering the previous 6 months worth of visits. When those records are delivered to your portal account, you’ll have access to a “Health Record” link where you can see that information.

If you have a health share plans such as Medi-Share, Samaritan Ministries, Liberty Health Share or others, you will be required to pay for services rendered on the date of the service. The fees will be according to our cash pay pricing. You may then submit your claims to your Health Share for reimbursement.
All copays are expected at the time of service. this is part of our contract with our insurance company. It is a violation of our contract to waive these fees or collect them at a later time. Please come to the office with your copay for each and every visit. Follow-up visits require copays as well.
You are responsible for all balances not paid by your insurance company.
