Do you need a birth certificate for Medical City Plano?

Aug 07, 2018 · The patient portal provides secure internet access to your personal health records at Center for Dermatology and Cosmetic Laser Surgery. Your user ID and password will be given to you by the staff during your appointment. Please keep this information in a secure location for future use. New passwords can only be requested in person at our office.

Certified Birth Certificates

When a child is born at Medical City Plano we submit the Birth Certificate information to the Bureau of Vital Statistics. Certified Birth Certificates can only be obtained through the Bureau of Vital Statistics, which is part of the Texas Department of State Health Services. The hospital does not have a certified copy on file.
