If you need copies of your medical record, please call 814-373-3446. The records can be prepared and sent to you via e-mail, US mail or fax. If you need to pick up the records at the hospital, the records will be available at the Cashier’s Desk inside Emergency Room entrance, Monday through Friday from 8:00am – 4:00pm.
