Doctor’s trips can be exhausting, and we feel that it is time for the tradition of the house-call to return to the forefront of medicine. If you live at an Assisted Living Facility (ALF) or if getting to the office is particularly burdensome for you, we have a team dedicated to you

WHERE DO WE PROVIDE CARE?

Currently, we provide medical care at Cedar Creek and Sunshine Gardens Assisted Living Facilities. We also provide care directly in your home if getting to the doctor’s office is exceptionally burdensome for you. If you are at another ALF not listed below or you wish to inquire whether we can provide homecare for you, please call our office

WHAT WE PROVIDE

Our HomeCare Services team work in pairs of a medical assistant and medical provider including physicians, physician assistants, and nurse practitioners. There is no difference to your cost and no extra charges will come to you. Your insurance covers your care in the ALF and home.

Two Ways to Create Your MyHealthHome Account

Enrollment Invitation – Bravera Health will send an email to the email address you provided upon registration to the hospital. Follow the instructions in that message to create your account.
