How do I register for my appointment at MD west one?

How many doctors are at MD west one?

With a team of 24 orthopedic, neurosurgery and spine surgeons and 17 physician assistants, MD West ONE has harnessed years of sub-specialized expertise under one practice.

OFFICE SERVICES

Payment for office services is expected at the time of each visit if you do not have insurance. An itemization of your charges will be given to you each time you are seen in our office. MasterCard, Visa, and Discover cards are accepted, as well as HSA account cards. If you have insurance coverage we will file your claim for you.

HOSPITAL SERVICES

If you should require hospitalization, we admit patients to Midwest Surgical Hospital, Methodist Hospital, Methodist Women’s Hospital, Methodist Physicians Healthwest, Nebraska Medicine, CHI Health Bergan Mercy, CHI Health Lakeside and CHI Health Lakeside ASC.

MEDICARE

We participate with Medicare and we will file your claims with them and with one supplemental carrier for all services rendered.

HEALTH PLANS

If your coverage is with one of the HMO’s and PPO’s with which we participate, we will file your insurance claim. Any co-payment amount must be paid at the time you are seen. Please bring any required referral forms with you or you will be responsible for your charges that day.

Responsible Party

Please note: The responsible party is always the patient, unless the patient is a minor or an incapacitated adult.

In the event I am unreachable, I authorize MD West ONE, P.C., to leave a message regarding my appointment time, changes or schedule information on my answering machine, voicemail or with the person answering the phone.

Policy Notice Receipt of Acknowledgement

I acknowledge that I was offered a copy and agree with the terms of the financial policy. *Link to the financial policy is at the top of the page.

IF APPLICABLE

I hereby authorize MD West ONE, P.C., to furnish third party payors with any information concerning the medical care, treatment and billings. I hereby assign to MD West ONE, P.C., all payments for medical services to be rendered to me or my dependents, and I authorize direct payment for such benefits to MD West ONE, P.C., by any third party payor.
