Welcome to your Patient Portal: Health Record Home. Health Record Home patient portal is a free and secure online home for your inpatient hospital health information. The portal allows you to view your medical records on any internet-enabled device and become a more active partner in your healthcare.

How to access hospital records?

When you sign-up for Health Record Home, you will be able to: 1 View and share your personal health record (PHR) related to your hospital stays. 2 Have electronic access to an updated list of medications, diagnoses, allergies, lab test results, patient history and other healthcare information related to your inpatient hospital visit. 3 Access your hospital discharge instructions for better after-discharge care. 4 Manage family members’ health records once you are authorized.

What is your username on Portal?

Your username will always be your email address. Each time you enter your Portal account, you also will need to enter your password and answer your security question for extra security.

Central Scheduling

Assists with selecting a family healthcare provider and/or scheduling appointments at family healthcare clinics.

Community Care Coordinators

Helps Medicare patients with two or more chronic conditions by becoming an extension of their primary care provider to support their healthcare goals, provide additional education, coordinate care (which may include medications, appointments and communication), and act as a resource to services available to improve quality of life.

Social Worker

Onsite staff person focused on guiding patients and their families with the next steps they may need in their care and to connect them with community resources that may be beneficial.

Dietitian

The nutritional status of our patients is a top priority at McKenzie Health System. We know that it takes skill and planning to provide a nourishing, well-balanced, and properly controlled diet.
