What can I do on the online health portal?

May 22, 2015 · Download your patient forms online through the patient portal When you come to our office for the first time as a new patient, we'll ask you to complete some initial forms, including an Authorization and Consent for Treatment form, if you were not able to download them from the patient portal in advance of your appointment.

Personal Interests

Dr. Van and his wife, Vicki C. Van, M.D., have two children, David and Lauren, ages 28 and 23. During his free time, Dr. Van enjoys playing golf, outdoor water sports, and family activities. His son, David, graduated Summa Cum Laude in Biomedical Engineering and Business and is attending LSU medical school.

Biography

Dr. David Van, a board-certified family physician, is the founder and medical director of McIntosh Trail Family Practice. Dr. David Van and his wife, Dr. Vicki Van started McIntosh Trail Family Practice in 1987. Since 1987, Dr. Van has seen his practice grow into one of the largest family practices in metropolitan Atlanta.
