The MyPortfolio Patient Portal at UMMC is a convenient and secure health-management tool you can use anywhere you have access to the Internet. With the MyPortfolio Patient portal, you can track your visit history, view lab and radiology results, review your discharge instructions, and much more to come in the future.
In your discharge information folder, there should be a single sheet of paper with a unique code for you to register on the UPMC Western Maryland Hospital Services Patient Portal on the bottom of it. You should have received an email to the address you provided when you registered.
Any individual who lives in the state of Maryland whose provider recommends medical cannabis as a treatment option for a qualifying medical condition is eligible to register with the Maryland Medical Cannabis Commission (MMCC) as a patient.
This includes information from your medical record at Western Maryland Regional Medical Center and the UPMC Western Maryland outpatient diagnostic centers. It does not include information from any physician offices.

While the registration processes and requirements differ for adult and minor patients, both are completed online. The online application form must be completed and submitted in one session; you will not be able to save and complete the application at a later time.
A registered patient is permitted to identify a maximum of two individuals, aged 21 years or older, to serve as caregivers. Caregivers are required to register with and purchase ID cards from the Commission. Registered caregivers in possession of a valid MMCC Caregiver ID card are able to purchase medical cannabis on behalf of their designated patients from licensed Maryland dispensaries and transport the legally obtained medical cannabis to their designated patients. A minor patient may have up to four caregivers, two of whom are a parent/guardian, and two additional adults over the age of 21 designated by a parent or guardian.
