How do I Register my child for the patient portal?

Registering for your child's Patient Portal is easy and only takes a few minutes. Just give us a call and we will be happy to register your child over the phone or you may ask a Front Office Representative at their next appointment. What Happens Next? Once your child is registered, you will receive a welcome email from South Lake Pediatrics.

Maple Pediatrics & Adult Primary Care is the practice of Michael J. Polisin, MD. Founded in 1990, Dr. Mike’s practice has been serving the Greater Summit Area for over 30 years. He and his staff have dedicated their lives to ensuring patients receive the best medical care around.

Does Maple Pediatrics accept insurance?

Maple Pediatrics is fee for service and we do not accept or participate in any insurance plans. Our practice is very much an old timey, boutique practice at your service.

Rapid PCR testing

We are currently doing rapid PCR testing in the office for Flu A, Flu B, Respiratory Syncytial Virus, and Covid -19 in the office. This testing is done with one nasal swab. Results are usually obtained within 1 hour. Our doctors will determine if this testing is appropriate for you child.

Can you use the Patient Portal for emergency care?

The Patient Portal is not intended for use in emergencies. If you require urgent or immediate medical care, call 911. Please call the clinic to schedule your child's first appointment. You may ask a receptionist to web-enable your account when you check in for your child's appointment.
