Dr. Barr performed right hip pin surgery following a fall in 2017. In May 2019, he replaced the hollow pins with solid pins when I complained the original pins were chafing my IT band. Soon thereafter, I developed ever-increasing pain. In February of 2020, an MRI suggested necrosis.

Once you have made your initial appointment, you may pre-register by downloading, printing and completing the forms below. Please be sure to bring them with you to your first visit. Completing the forms prior to your visit will help speed up the new patient registration process.

Established Patients

Any registration and history information can now be entered or updated via our Patient Portal. If you have not activated your Patient Portal account, please do so as soon as possible. If you do not have an activation code, contact our office and we will provide you with your unique code and the necessary steps to set up your Patient Portal account.
