Our Patient Portal gives you secure online access to some of your health information found at Madison Memorial Hospital. This will allow you to access your health information by clicking on the Patient Portal button below whenever you have internet access.

An unsuccessful enrollment request does not preclude participation in the patient portal, only that a team member must review your information to correctly link your clinical account.
