How to request a release of information from UHS?

You can use the Patient Portal to request that UHS send your medical records to you, another medical provider/facility/institution, or insurance company. Login to your account and click the "Forms" tab. Under Section A: UHS Forms, click "Release of Information UHS.". Complete and submit the Release of Information form.

Do minors have their own email address?

Patient Portal accounts must contain a unique email address. Minor children typically do not have their own email address. Due to HIPAA regulations and patient confidentiality policies, these accounts cannot be created at this time.

NEW! UHS is now offering video visits for COVID-related concerns or cold-like illnesses. You can self-schedule your appointment 24/7 by logging into www.myuofmhealth.org and clicking on "visits" or call 734-764-8320 to schedule during business hours.

On July 30, 2021 the university announced that COVID-19 vaccinations are required for all University of Michigan students, faculty and staff by the start of the fall term on Aug. 30.

How to Get Health Care

For patient safety, appointments are required for all clinic visits. Patients should call 734-764-8320 to be scheduled and to be screened for COVID-19.

Domestic Student Health Insurance Plan (DSHIP) 2021-22

Students, do you need health insurance? If so, please consider this plan, which was developed with students' needs in mind. Enrollment for the Winter 2022 open enrollment period is now open, with coverage dates 1/1/22 to 8/23/22. The final day to enroll for this coverage period is January 31,2022. The cost for an individual student is $1,242.
