MyChart is a secure online service that gives you, or your substitute decision maker, access to your medical records, from various participating hospitals.
Some data availability is delayed due to the potential sensitivity of the data and/or the need for clinical review and/or quality assurance, prior to disclosure.
Currently the data available in MyChart is hospital-based and, at this time, does not include information from your primary care provider’s/family physician’s electronic medical record.
You, or your substitute decision maker will be required to provide an Ontario health card and a valid email address
If you would like to make a follow-up appointment with a physician at Louisiana Women’s Healthcare you have already seen, you can do so through the MyOchsner app.
