What is MyChart online?

MyChart is a secure online service that gives you, or your substitute decision maker, access to your medical records, from various participating hospitals.

Why is data availability delayed?

Some data availability is delayed due to the potential sensitivity of the data and/or the need for clinical review and/or quality assurance, prior to disclosure.

Does MyChart include primary care?

Currently the data available in MyChart is hospital-based and, at this time, does not include information from your primary care provider’s/family physician’s electronic medical record.

Do you need an Ontario health card to apply for a substitute decision maker?

You, or your substitute decision maker will be required to provide an Ontario health card and a valid email address
