We welcome new patients! Please contact us to schedule an appointment. The Welcome Center staff will reach out to you to complete a new patient orientation as your new patient appointment approaches.

Existing Patients

We are here for you, 7 days a week. During this time, we are limiting onsite appointments and are providing our patients with telehealth visits. If you do not have health insurance enrollment assistance is available over the phone. Sliding fee (discounts) may be available. No one will be refused care because they cannot afford to pay.

What is a CHW in Lowell?

CHW s are public health workers who apply their unique understanding of the experience, language and/ or culture of the population s they serve in several roles to eliminate health disparities. Lowell Community Health Center has over 60 CHW s and Peer leaders working in various departments.

What is a Lowell CHC?

For over fifteen years, Lowell CHC has worked with local Faith-Based Organizations ( FBO s) to bridge a number of barriers individuals face in finding and retaining regular health care and to reduce health disparities. Health Center Community Health Workers ( CHW s) collaborate with a network of over 40 faith-based organizations including Khmer (Cambodian) and Laotian Buddhist Temples; African community churches, mosques and Latino and Portuguese houses of worship in Greater Lowell. At each visit to a FBO, Health Center staff usually connect with 100-150 individuals, providing health education and information on how to access programs and services at the Health Center and in the community. Additionally, the health center hosts a Faith leader’s luncheon each year to celebrate the diverse Faith leaders in the greater Lowell Community

With locations available throughout the Merrimack Valley, patients needing blood tests, imaging services, and prescreening and outpatient testing will find a Lowell General Hospital Patient Service Center close, convenient and accessible to their home or work.

What is the new patient portal?

Our new patient portal provides students with the ability to electronically submit health forms and immunization documents required by Massachusetts law. Students will also be able to view portions of their medical record, access health education information, and receive secure messages from their Wellness Center medical provider.
