Who is Tiffany from Little Rivers?

Tiffany is a Licensed Clinical Social Worker (LICSW) and a Licensed Alcohol and Drug Counselor (LADC) in the State of Vermont. She completed her master’s degree at the University of Vermont in Transformative Social Work. Her focus during her master’s program was centered on individuals who have experienced trauma. Her post graduate employment as a therapist, included treating individuals with co-occurring substance use and mental health disorders at the residential program Valley Vista. At Little Rivers Health Care, Tiffany currently serves as the Program Coordinator for the Medication Assisted Treatment program in addition to treating individuals in the Behavioral Health Program.

Where is Alex from Little Rivers?

Alex is a Family Nurse Practitioner who has lived in northern Vermont for the last 6 years. She received her Bachelor’s Degree at Middlebury College and completed her Masters of Science in Nursing at Simmons College in 2018. She worked for many years as a registered nurse in women’s health. She is thrilled to be part of the Little Rivers Health Care team caring for members of the community of all ages and through all stages of their lives. Outside of work she enjoys spending as much time as possible outdoors with her family and dogs.

Who is Fay Homan?

Dr. Fay Homan has been with Wells River Clinic since 1993. She enjoys the full scope of family medicine, from obstetric care to geriatrics. Teaching the next generation of family doctors is an important part of her practice, and you’ll often meet medical students from UVM and Dartmouth with her in Wells River. She also enjoys international medicine, and has travelled to Honduras, Kosovo and Malawi. She serves on the board of the Vermont Academy of Family Physicians and the Primary Care Advisory Group for the Green Mountain Care Board. She got her medical degree from the University of Vermont and did her Family Medicine residency in Tacoma, Washington. She has been honored as the Vermont Family Physician of the Year and as Family Medicine Teacher of the Year.

LRMC is open and our primary care providers are dedicated to helping you receive the care you need. Because our top priority is the well-being of our patients, visitors and staff we are taking precautions to keep everyone safe and protected from possible exposure to COVID-19. All providers, staff, patients and visitors must wear facemasks when in the facility. If you do not have a facemask, one will be provided for you. We are rearranging our waiting rooms to encourage physical distancing. At some of our sites, you may be able to wait for your appointment in your car.
