What time does the office open for doctor appointments?

For doctor appointments, please contact our office during regular phone hours (8:15a.m.-4:00p.m.) Monda y through Friday. Patients with urgent problems who wish to be seen should call the office and visit with their nurse for scheduling an appointment or other directions.

Where to go if you have a medical emergency?

Go to the emergency room if you think you are having a medical emergency. You can also go when your Doctor’s Office and Urgent Care clinic are closed. You may have to wait a long time to receive care.

What is an Internal Medicine Physician?

An outpatient internal medicine physician, also known as an Internist, is a primary care physician (PCP) who specializes in care of adults across the spectrum from health to complex illness.

What is a Geriatrician?

A geriatrician is a physician who specializes in the care of older adults: promoting health, independence, and quality of life while managing comorbid chronic conditions.

What is a Nurse Practitioner (NP)?

A nurse practitioner (NP) is a clinician that blends clinical expertise in diagnosing and treating health conditions with an added emphasis on disease prevention and health management.

The Doctors

David Goldstein, M.D. FCCP Dr. Goldstein is a 1976 graduate of Brandeis University, and he earned his medical degree at the Albert Einstein College of Medicine in 1980. He served his internship and residency at Jacobi Medical Center in New York.

Nurse Practitioners

Darlene Fontanazza graduated from Wagner College in 1990, and then went on to complete her nurse practitioner training at Columbia University of New York. She joined Pulmonary Internists in 1993 as a Nurse Pracitioner after working as a Critical Care Nurse at JFK Medical Center. She serves as a preceptor for nurse practitioners in training.

Physicians Assistants

Rachel joined our practice in 2019 after working in a family medicine practice. She graduated from Yeshiva University's Stern College for Women with a degree in biology. She continued on to receive her masters degree in Physician Assistant Studies at Touro College in Bay Shore, New York.

Respiratory Therapists

Karen received her degree in respiratory therapy from Union College and her Bachelor of Science from Kean University. As a registered respiratory therapist, she has performed numerous duties within the pulmonary field since 1979. She was employed by Muhlenberg Hospital in Plainfield for 30 years before transferring to JFK Medical Center in Edison.

What is patient portal?

The patient portal allows you to communicate with our clinics online. You can use it to view or request appointments, refill prescriptions, view your billing statement, and more. If you do not have a username and password, please call our office to receive them.

How long does it take to get a referral from CJIMA?

CJIMA requires 3-5 business days to process referrals or authorizations. Please call us on our referral line at 732-828-0002, Option 2, and provide: Patient’s Name, Date of Birth, Call Back Number, Specialist’s Name, Appointment Date, and Reason for Visit.

A third-party vendor handles all CJIMA’s medical record requests and can be reached at: Med Request Solutions, P.O. Box 23, South River, NJ 08882. Tel: 800-483-6040.
