As a MyChart user, your role in maintaining the security of your medical information is: 1) Changing your password on a regular basis, and 2) Keeping your login ID and password confidential.
For access to the LMC MyChart Account of an adult patient or a minor patient ages 16-17, the patient and the proposed proxy complete the “Proxy for Adult Application.” Proxy access to an adult patient’s MyChart Account will automatically terminate three years after establishing proxy access.
