What is Legacy Patient Portal?

My Legacy patient portal is a website that allows you to interact and communicate with your health care team via the Internet. The portal services are available to you at anytime and allow you to acces your medical records along with a variety of other functions.

How long are Legacy consent forms good for?

Once completed, these forms will be good for a year, but we wanted to let you know you will be asked to complete it. You may also download, complete, and bring the forms with you to your next appointment.

Can Legacy access my health information?

To ensure that your private health information is safe from unauthorized access, the My Legacy portal is hosted on a secure connection. The application is accessed via an encrypted, password-protected login that only you have access to. Legacy staff cannot view this information, and therefore cannot accidentally provide your username and password to unauthorized people. Your account cannot be accessed without the password, nor can changes be made to your account profile.

What is legacy imaging?

Legacy Imaging Services is a leader in advanced technology, preferred by patients for its customer service and caring staff. All our physicians are board-certified and fellowship-trained in their areas of specialty and all technologists are registered in their specific areas of focus.

Where is the parking lot in the Medical Plaza?

If your appointment is in the outpatient imaging suite (19260 SW 65th, Suite 165) parking lot is located in front of the Medical Plaza Building 2.

Where is the registration desk at the hospital?

If your appointment is in the main hospital (19300 SW 65th Ave), the registration desk is located to your left when you walk in the main entrance.

Is Legacy Meridian Park accredited?

Legacy Meridian Park is accredited by The Joint Commission and the American College of Radiology (ACR) in CT, MRI, Ultrasound and Mammography. The Breast Health Center was named as a Breast Center of Excellence by the ACR, and has also received accreditations from the National Accreditation Program for Breast Centers (NAPBC) and Mammography Quality Standards Act (MQSA).

What is Legacy Portal?

The Legacy Portal gives providers and medical staff quick access to some of their most-used resources and tools, including Epic, PACS and other Citrix applications, news highlights, eDocTalk, and quick links to clinical tools, schedules, training, education and more.

What is Legacy Health?

Welcome to Legacy Health's primary care clinics, the first step to better health. Our clinics are certified patient-centered medical homes. A medical home clinic means that you and your family are at the center of our care, surrounded by a team that consists of your primary care provider, nurse, medical assistant, social worker, counselor and clerical staff. This team is focused on your health needs.

What does it mean to be patient centered?
