My Legacy patient portal is a website that allows you to interact and communicate with your health care team via the Internet. The portal services are available to you at anytime and allow you to acces your medical records along with a variety of other functions.
Once completed, these forms will be good for a year, but we wanted to let you know you will be asked to complete it. You may also download, complete, and bring the forms with you to your next appointment.
To ensure that your private health information is safe from unauthorized access, the My Legacy portal is hosted on a secure connection. The application is accessed via an encrypted, password-protected login that only you have access to. Legacy staff cannot view this information, and therefore cannot accidentally provide your username and password to unauthorized people. Your account cannot be accessed without the password, nor can changes be made to your account profile.

If your appointment is in the outpatient imaging suite (19260 SW 65th, Suite 165) parking lot is located in front of the Medical Plaza Building 2.
