2021 Survey Results are Live

2021 Leapfrog Hospital and ASC Survey Results are live. Survey Results will be refreshed within the first 5 business days of each month to reflect Surveys submitted or re-submitted between August 1 and November 30, and previously submitted Surveys that are corrected by January 31. Survey Results are then frozen from February to July 25.

Monthly Data Verification Messages

Primary, Secondary, and System Contacts are reminded to look out for Data Verification Messages, which are emailed within the first 5 business days of the month following submission. Add the @leapfrog-group.org and @leapfroghelpdesk.zendesk.com domains to your safe sender list to ensure messages are received.

Prepare to Submit a Survey

In 2019 and 2020, over 2,300 hospitals submitted a Leapfrog Hospital Survey. Please review the information on these webpages before you begin. These resources will help you to successfully complete the submission process.

Alignment with Other National Programs

Many of the measures included in the Leapfrog Hospital Survey are in use by other national agencies and organizations including the Centers for Medicare & Medicaid Services (CMS), the CDC's National Healthcare Safety Network (NHSN), and The Joint Commission.
