Your Primary Care or Specialty provider’s office will start the process of enrolling you in the patient portal. A paper copy of the Portal Activation will be given to you at the time of registration or a letter will be mailed to you.
The staff members in the Medical Records department will be able to walk you through the process to sign up to the portal. Online self-enrollment is available if you are 18 and over. You will need your medical record number in order to successfully register online. Online self-enrollment is currently not available for caregivers or guardians.

To delete your account, click “My Account” in the upper right-hand corner of your FollowMyHealth homepage. Then, click on “Preferences.” Then, click “Delete your UHR” under “Account Preferences.” The system will prompt you to confirm once more. If you so choose, click “Yes,” and your account will no longer be active.
