Lakewood Family Medicine is located on Holland’s north side, on 120th Ave between James Street and Lakewood Boulevard. Our offices provide a comfortable and spacious environment with a separate children’s waiting area. Laboratory services provided by Holland Hospital are also conveniently located on site.

Testing for SARS-CoV-2 (COVID-19)

COVID-19 testing is available by appointment only. Testing for pre-op clearance, travel, and antibody testing is done in the office with a negative COVID-19 questionnaire.
