Log into FollowMyHealth®. Go to "My Account," then go to "My Connections." The providers listed are LRH providers that you have seen or that you have an appointment with in the future.
FMH offers a wide variety of support from within the portal. From the Home Screen, select My Account and from the drop down select Support. Once the Support Information opens, you can click Knowledge Base for Frequently Asked Questions or click the User Manual for step-by-step instructions on using the portal.
