To be eligible for Lake Regional's Chronic Care Management program, patients must have two or more chronic conditions, such as arthritis, COPD, heart disease, cancer, diabetes, obesity, depression or some other ongoing medical problem. Participants also must have a Lake Regional primary care provider and be enrolled in Medicare.

What is primary care?

Primary care clinics provide sick visits for such needs as a sore throat or fever, as well as management of chronic conditions, such as diabetes and high blood pressure. Primary care clinics also provide well-child checks, sports physicals and preventive medicine for adults.

Does Medicare cover diabetes?

Medicare covers a full range of preventive services to help keep you healthy and help find problems early, when treatment is most effective. These services include cancer screenings, cardiovascular screenings, vaccinations, diabetes prevention and more. Ask your primary care doctor which services are right for you.
