If you need to request your medical records be transferred to or from another hospital or physician's clinic, please download the forms below and return them to the Medical Records department by fax or in person :
Located in the Atrium on Oak Park Campus
Patients pre-admitting for surgery complete their registration, nurse interview, blood work, EKG, and chest x-ray all in one area. If a patient needs financial counseling, they can also complete a Medicaid Financial Assistance application in the patient access center.
