If there is no data included with the description of the measures below, it may be because your facility doesn't offer that particular service or treatment. Some data is reported every month or quarter and some is reported once a year. Occasionally, a measure is no longer used.
One measure is not an indication of a facility's quality. Sometimes a smaller population can make a measure move pretty drastically from quarter to quarter, so don't be alarmed if you see a dip or a spike.
Physician offices or other health care providers may request a copy of medical records by faxing the request on office letterhead to (256) 386-4687. Patients may also request a copy of medical records by mail or in person.
If the patient is under the age of 14 , physically or mentally incapacitated or deceased, a legally designated representative may complete the necessary forms on behalf of the patient and submit the request.
Once Medical Records receives a request, it takes five to seven business days to process paper copies, three business days to process electronic copies and 30 days to process records currently stored on microfilm.
If medical records are needed for continuing care, there is no charge when records are faxed directly to your physician or the facility providing treatment. No Charges to veterans or active duty military personnel with Military Identification.
