To obtain copies of your Our Lady of Bellefonte Hospital medical records, please contact CIOX at 1-866-625-7130. King's Daughters does not have copies of patients' OLBH records unless the patient has made specific arrangements to have the records transferred to us.

What does King's Daughters do with images?

Following a request, the Radiology Imaging Resource Center will send the images to King s Daughters. The images will be uploaded to our PACS system for continuation of care. If the patient requests images to be sent to their address, the Radiology Imaging Resource Center will deliver/mail the image disk to the patient and the patient will need to give these to their provider directly.

How long does it take to get a copy of a medical imaging study?

Depending on the records/imaging studies you are requesting, it may take up to 21 days to process your request. Please be as specific as possible when completing the Access Form. If you need your imaging studies supplied on a disc, please note this on the form.

Keep track of your health, including current issues, medications, growth charts, allergies, immunizations and medical, surgical and family history.
