Primary Care Provider (PCP) – Each patient has the right to select their own PCP. A PCP can be a doctor or a nurse practitioner. You will benefit from an ongoing relationship with your PCP who will provide complete and continuous care.

W elcome to our Patient Portal, a secure and confidential means of communication between you, our valued patient and our practice. We will use the Portal to share important updates with you regarding your health information. You may request copies of your health information at any time, whether it's an order for lab-work, recent test results, medical records, even copies of imaging!
We will normally respond to non-urgent inquires within 24 hours but no later than 3 business days after receipt. If you have not heard from us within 3 business days, please call the office at (847) 663-9700 to check the status of your request.
