If you received care at Kootenai Health (admitted as a patient, had blood drawn at the Kootenai Health lab, visited the emergency department, or had an MRI, CT or CAT scan at Kootenai Health).

Kootenai Health’s health portal is completely secure, so you can be confident that your private information is protected. Only you, or an authorized family member, can access your information through the portal.

When do you sign an authorization to release?

There are several option to complete a signed authorization to release records. You may come to our office between the hours of 8am-4.30pm to sign an authorization to release records. Please bring your ID with you. If this is for a child, the parent or legal guardian will need to sign.

Do you have to be a patient to complete a CHA physical?

Please note that in order for our providers to complete the form, you must be a current patient of CHA and have had a Physical Exam completed by our provider in the current year.

Do you need to sign a fax form for CHA?

If you are wanting CHA to fax this form to another location for you, such as to your employer or school, you will need to sign an authorization form giving CHA the permission to release this information.
