What is a Notice of Privacy Practices?

A Notice of Privacy Practices is provided to each patient at their first appointment. Patients are asked to sign an Acknowledgment of Receipt of Notice of Privacy Practices at that time. If a patient chooses not to acknowledge the Notice of Privacy Practices, we will not condition treatment on the patient’s acknowledgement.

What is the privacy rule?

The Privacy Rule gives you rights with respect to your health information and sets limits on how your health information can be used and shared with others. The Security Rule sets rules for how your health information must be kept secure with administrative, technical and physical safeguards.

What Insurance Do We Accept?

Jordan Valley accepts Medicaid, Medicare and most private insurance. You can call our office with any questions—just make sure you have your insurance card handy! If you need information about your coverage, call the number on your insurance card and talk to your carrier.

Missouri Medicaid

We accept Missouri Medicaid. If you have questions or would like to apply for Medicaid, we can help you complete your MO HealthNet application.

Slide Program

We want our services to be available to as many people as possible. Our sliding fee program is for patients who do not have health insurance. Based on your income, you will pay a fee when you visit, similar to a co-pay.
