What is the MyChart Patient Portal?

The MyChart Patient Portal is an online tool that provides medical information about care provided at Johns Hopkins All Children’s and connects you to your health care team.

When can a parent access MyChart?

When the patient is age 12 or younger: Parents/guardians can request a proxy account and will have full access to the child’s MyChart information. For patients ages 13 –17: Parents/guardians can continue to use MyChart to share messages with some of their providers and view or pay bills.

When users try to log into HealthLINK, after putting in their user name and password, they will be brought to a page prompting them to receive a security code either through text, (if they have a cell phone number listed in their account) or by email. Enter the security code on the next screen in order to access the USFHP portal in HealthLINK.

The form must be completed by the patient or patient representative and clearly state the dates of service, the specific type of record (s) desired and all other information indicated on the form.
