Looking to obtain a copy of your medical records?

You may request the release of your medical records by printing and completing the following form:

Please allow seven to 10 days for Ciox, the firm who handles these requests to process. Virginia law permits a charge for your records; Ciox will invoice you directly. Rates are $0.25 per page plus postage and handling. Prepayment may be required before records are mailed out.

Facility or Doctor

If you are a facility or a doctor’s office requesting records on a patient currently being treated, or you are a patient requesting records be sent to a physician for continuation of care, please fax a completed release form to (855) 226-6070. Records for continuation of care will be expedited as needed and there is no charge.
