Obtaining a copy of your health records, or having your health record sent to another health care facility, is easy. To start your request, simply download, print, complete and sign the applicable NSU Health Authorization for Use and Disclosure of Protected Health Information Form (“Authorization Form”) listed below.

How We Bill Your Insurance and You

About 5 days after your visit we will send your insurance company a bill for the services you received. You may receive a letter from your insurance or from us if your insurance company is requesting additional information from you. Please respond to your insurance company at the number they provide or to our team at 954-276-5501.
