Are these practices or providers part of Johns Hopkins Medicine?

Follow these steps to register: Go to www.shwcportal.jhu.edu/PyramedPortal Select “Click here to register” Enter your Hopkins ID (found in SIS ), Hopkins email address and date of birth You will receive an email from Portal with a link to create your password After clicking the link, you will be prompted to create an 8 character password

For access to patients age 17 and younger, use your adult account to request access to a child's records. Adults (age 18 and older) can use their MyChart account to request another person to access their MyChart information to help with their health care. Proxy access can be stopped at any time by the proxy or the patient.

Register Your Account

Before you can access the Health WebPortal, you will need to register your account. Follow these steps to register:

Complete Online Health Forms

In addition to completing a paper pre-entrance health form, students must complete the required pre-entrance online health forms through the Health WebPortal. To access the forms:

Send and Receive Secure Messaging

The Health WebPortal’s secure messaging feature provides students with a HIPAA compliant means of communicating with providers that will protect your privacy. Please do not send secure messages for urgent matters.

How old do you have to be to use MyChart?

Parents and legal guardians can request proxy access to their child's MyChart information until the child is 18 years old. The information available to the child proxy through MyChart is limited for children aged 13-17.

How long does a prepaid card last?

If you don't use the code within 45 days, it will expire. If you have difficulty using your activation code prior to its expiration date, please call 800-318-4246 for technical support, or contact your provider's office to request a new activation code.

Can I download MyChart?

Yes. You can access the full web-based version of MyChart from your mobile device. You can also download a free MyChart app from the iPhone App Store or Android Market by searching for MyChart. After download, select Johns Hopkins Medicine from the app's phone book.
