The Patient Portal is a secure, online service providing patients of The Urology Group with personalized access to their medical records. This free service offers convenient, 24-hour access, allowing you to manage your care and receive information about your health and treatment when the time is right for you.
Provide The Urology Clinic with your personal email address. We will provide you with a Patient Portal Welcome Letter that will be sent to the provided email. This letter will contain your temporary password to access the Patient Portal. You can also request a print out of the email that will be sent.

Log in to your patient portal account and click the “Health Forms” tab to view and complete your forms.
