The JCMH Patient Portal is being offered to all JCMH patients that are over 18 years of age that have had services at JCMH or a JCMH Clinic. Health Information is also via the JCMH Patient Portal is also available to parents and legal guardians of children that are 0-12 years of age. Some limitations will apply.

At this time, JCMH only offers access to a child’s medical information via the JCMH patient portal for children between the ages of 0-12 years old.
In order to have Proxy Access to another adult's portal, the Proxy Request Form must be filled out. If you are the Power of Attorney for Healthcare decisions or Healthcare Proxy, the Proxy Access Form must be filled out and legal documentation presented in order to gain portal access for that person.
Proxy access may be revoked at any time by patients age 18 and over by filling out the Proxy Revocation Form.
