The IOM report discusses policy issues, the healthcare safety-net system, forces affecting the system, and the future viability of the system. Additional discussion about the safety-net system is found in Part 4. The following IOM reports focus on the importance of leadership in health care.
While the Institute of Medicine (IOM) report makes special mention of the role for APRNs in primary care (see Box 2-1 ), the report's recommendations are not limited to those settings, but encompass the full continuum of health services in many health organization and community settings.
Every nurse must be educated on the key messages of the IOM report and become involved in moving these recommendations forward as well as in educating others on what needs to be done.
The Department of Health and Human Services (HHS) requested that the IOM develop and recommend 12 leading health indicators and 24 objectives to assist HHS in its determination of leading indicators and objectives for Healthy People 2020 (2011g).
Overall, pathologists ensure laboratory quality so that diagnostic testing is safe and accurate. They are responsible for the diagnostic accuracy of every test. Often, they guide primary care and other doctors, determining the right test, at the right time, for the right patient.
A pathologist studies fluids, tissues, or organs taken from the body. Pathologists often work with a surgically removed sample of diseased tissue, called a biopsy. The pathological examination of an entire body is an autopsy. Pathologists are often involved in the diagnosis of illness.
Having strong communication skills helps doctors understand their patients' symptoms, which makes it easier to effectively diagnose patients and recommend the best course of treatment. Doctors must also be able to communicate with nurses verbally and through charts to ensure patients are taken care of properly.
The importance of an accurate diagnosis can't be underestimated, because diagnostic errors cause delays and mistakes in treatment that can be fatal. The SIDM offers a patient toolkit and a patient checklist jointly developed with the National Patient Safety Foundation and the Cautious Patient Foundation.
It underpins every aspect of patient care, from diagnostic testing and treatment advice to using cutting-edge genetic technologies and preventing disease. Doctors and scientists working in pathology are experts in illness and disease.
The American Osteopathic Board of Pathology also recognizes four primary specialties: anatomic pathology, dermatopathology, forensic pathology, and laboratory medicine. Pathologists may pursue specialised fellowship training within one or more subspecialties of either anatomical or clinical pathology.
The RESPECT model, which is widely used to promote physicians' awareness of their own cultural biases and to develop physicians' rapport with patients from different cultural backgrounds, includes seven core elements: 1) rapport, 2) empathy, 3) support, 4) partnership, 5) explanations, 6) cultural competence, and 7) ...
Effective: Providing services based on scientific knowledge and best practice. Patient-centered: Providing care that is respectful of and responsive to individual patient preferences, needs and values, ensuring that patients' values guide all clinical decisions.
A patient has the right to information from his or her doctor in order to make informed decisions about his or her care. This means that patients will be given information about their diagnosis, prognosis, and different treatment choices. This information will be given in terms that the patient can understand.
Why Is Diagnostic Testing Important? Accurate and efficient diagnostic procedures are crucial for doctors to create effective treatment plans that allow patients to recover as quickly as possible with the least amount of complications.

Clinical diagnosis. A diagnosis made on the basis of medical signs and reported symptoms, rather than diagnostic tests. Laboratory diagnosis. A diagnosis based significantly on laboratory reports or test results, rather than the physical examination of the patient.
A pathologist is a physician who studies body fluids and tissues, helps your primary care doctor make a diagnosis about your health or any medical problems you have, and uses laboratory tests to monitor the health of patients with chronic conditions.
Clinical pathologists are also physicians who specialize in the field of diagnosis and with the help of laboratory tests, they manage diseases on the basis of the researches.
Who does the autopsy? Autopsies ordered by the state can be done by a county coroner, who is not necessarily a doctor. A medical examiner who does an autopsy is a doctor, usually a pathologist. Clinical autopsies are always done by a pathologist.
Pathologists typically need a bachelor's degree, a degree from a medical school, which takes 4 years to complete, and, 3 to 7 years in internship and residency programs. Medical schools are highly competitive.
This report continues the examination of safety issues and relates to the recommendations found in To Err Is Human ( IOM, 1999 ). In the process of giving health care, providers need to: (1) access complete patient information; (2) understand the implications of environmental factors such as waiting time to receive care, bed availability, and so on; (3) use information about infectious diseases to decrease patient risk; and (4) appreciate the implications of chronic illness and how these may affect care needs. Each of these elements depends on accurate, timely, and accessible information in the form of a comprehensive electronic health record/electronic medical record (EHR/EMR). The EMR/EHR supports the implementation of best practices and evidence-based care and facilitates standardization of care where appropriate. Additional discussion about informatics is found in Part 4.
The first report in the Quality Chasm series, To Err Is Human ( IOM, 1999 ), sounded the alarm for healthcare providers and for consumers that there are too many errors in the healthcare system. Preventing Medication Errors takes the next step by proposing a methodology to prevent medication errors ( IOM, 2006d ). The report focuses on several aspects of medication errors. First is the drug development system, along with regulation and distribution issues. Second, the report reviews the literature on the incidence and costs of medication errors and prevention strategies.
The purpose of this commission was to advise President Clinton about healthcare delivery system changes related to quality, consumer protection, and the availability of needed services ...
The focus of the report is not public health, but instead how the healthcare system performs in providing personal health care (health care for the individual). In addition, the report discusses health care from a broader perspective than the performance of individual providers such as hospitals.
Three criteria are used to select the priority areas: (1) impact or extent of burden, (2) improvability or extent of the gap between current practice and evidence-based practice, and (3) inclusiveness or relevance of an area to a broad range of individuals.
The increase of chronic conditions in the United States has had a major impact on the system and is an important consideration in identifying the priority areas. More people are living longer and with chronic illnesses, mostly because of advances in medical science and technology. Many of these patients also have comorbid conditions that increase the complexity of their problems and require more collaborative health care. Three criteria are used to select the priority areas: (1) impact or extent of burden, (2) improvability or extent of the gap between current practice and evidence-based practice, and (3) inclusiveness or relevance of an area to a broad range of individuals.
The quality report should assist policy-makers and should also be accessible and relevant to consumers, purchasers, providers, educators, and researchers. The design of the annual quality report builds on the definition of quality used in the various IOM reports.
The goal of the IOM Future of Nursing report, titled “ The Future of Nursing: Leading Change, Advancing Health ,” was to provide a prescription for nurses to facilitate the nation’s shift from hospital-based services to a system focused on prevention and wellness in the community. It was a bold move that has influenced nursing education and practice for the past decade.
Significant strides have been made related to increasing the number of nurse practitioners who can work as primary care providers — an IOM Future of Nursing goal aimed at increasing access to care.
Students also often struggled to complete their clinical rotations during the day because they were juggling jobs with the demands of nursing school. This prompted the schools’ clinical partners to allow more flexibility when scheduling hours for students.
Health IT, when well designed and implemented, is a tool that can help health information flow in ways that allow for improvements in patient health and safety. Whatever the drawbacks to health IT systems, the evidence suggests that health IT has raised the floor on safety.
Since then, whether health IT actually improves patient safety has remained an open question. The nation has seen widespread adoption of health IT as a result of the Medicare and Medicaid EHR Incentive Programs. With that increase in adoption, there should be more and better evidence on the actual impact of health IT on safety.
