Be sure to have your photo id, insurance card, list of current medications, relevant medical or surgical history, and any important questions you may have for your doctor.
If for any reason you are unable to keep your appointment please notify us 24 hours prior to your scheduled visit. If you are unable to cancel or reschedule your appointment 24 hours prior to your visit you will be charged a $50 cancellation fee. Please call our office for immediate assistance.
Co-pays and deductibles are due upon arrival for your appointment. There is a $40 charge for all returned checks, including non-sufficient funds. There will be a $15 charge for each form brought in for completion. Payment is due when the form is presented. If you are late for your appointment, we may have to reschedule your appointment.
