What insurances does Integrative Family Medicine work with?

Integrative Family Medicine is billing insurance for the following plans: Blue Cross Blue Shield (all plans, in state and out of state), United Healthcare, Humana , Medicare, Medicaid (Iowa Total Care starting 5/26/21), and Tricare. We are pending approval with Amerigroup, and Aetna/Cigna. If you are curious whether your insurance is in network, please call your insurance company or our billing office at 515-243-2057. When calling your insurance company, give them the name “ Integrative Family Medicine ” and provider “Sarah Lee, ARNP” for CPT codes 99203-99205. Our office visits and some treatment options are generally covered by insurance, pending your benefit plan. HSA and FSA accounts are accepted for co-pays, supplement purchases, and other treatment services such as IVs and injections. If you have a high deductible plan, it may benefit you to pay out of pocket for the cost of the office visit and submit to insurance on your own, as our billing rates are higher than our cash rates and patients may be left with additional amounts owed over our cash prices if Integrative Family Medicine completes its billing process.
