For Established Patients

You can access our patient portal (Your Health File) by clicking here. You must first activate your account by clicking the orange “Activate Your Account Here” button and using the username and password that was printed for you at your visit.

For New Patients

Please call our office at 303.805.1264 and make your New Patient Consultation (to establish care). We can send you a new patient packet via email/fax/US mail or you can click below on the packet for the appropriate gender.
