What is the coronavirus (COVID-19)?

COVID-19 is a respiratory illness that can spread from person to person. The virus that causes COVID-19 is a novel coronavirus that was first identified during an investigation into an outbreak in Wuhan, China and that has since spread globally.

How do I get the COVID-19 vaccine?

IHCSCV is following the CDC guidelines for the distribution of vaccines, prioritizing distribution to established patients, American Indians, Alaskan Natives and the community at large – ages 5 and over. Contact your health care provider to schedule your vaccination or call the IHCSCV Vaccination Hotline at 408-960-2050.

How do I get tested for COVID-19?

COVID-19 tests are being administered by appointment, only. If you or your family members are experiencing possible COVID-19 symptoms, like cough, fever, or shortness of breath – or, have been exposed to any ill contacts – please, do not come to the clinic.

Current Scope of Service

In order to keep our patients, staff, and community safe, in-office visits are being kept to a minimum and some of our hours have changed. Did you know that most services can now be accessed via telehealth? This means you could have a Medical, Dental, or even Counseling appointment by phone, tablet, or computer.

Patient Portal – Telehealth

We encourage all of our members to sign up for IHCSCV’s Patient Portal. This can help you have faster, more direct, communication with your health care team and make it easier for you to visit with your provider by phone and over the internet. Transformation Services

IHC provides limited transportation on certain days to all sites. Other options are: VTA/Outreach/Pre-arranged transportation by health plans.

How to Make an Appointment

Please call the clinic number to set up an appointment, call our phone operators, or send a message through our Patient Portal.

Program Eligibility

If patients are not eligible for any coverage, they will be placed on the medical self-pay Sliding Fee Scale or asked to pay full-fee. Depending on the level of income and their household size, patients will be asked to pay 40%, 50%, 75% or 100% of the total visit charges. If clients cannot afford, they will be billed for the visit.
