If you would like someone to have access to your records to help manage your care, you can sign a release to authorize a trusted relative to be set up as a proxy. In the future we hope to add additional functionality that allows you to add additional information related to your health to your My Portal (Patient Portal) account. If you do so, you'll have your information in a secure place that can be accessed anywhere you have Internet access.

Children under 12 years of age and dependent adults who have a Dignity Health doctor may have a My Portal (Patient Portal) account with an adult as the proxy. Accounts are not available for adolescents 12 – 17 years of age. For a dependent adult, they will need to sign a release, or you will need to provide a copy of the Medical Durable Power of Attorney before an account can be created for the dependent adult.
