  • Signed application form
  • Fee (if applicable)
  • Copy of a valid government-issued photo ID
  • Two items showing proof of residency
  • Designated caregiver information and caregiver card fee (if applicable)

  • Once logged in, select “Online Services” in the top right corner
  • Under Activities, select “Provisional Registration”

What are the rules for medical marijuana in Illinois?

Use is prohibited in:

  • Any public place, such as streets or parks
  • In any motor vehicle
  • On school grounds, with the exception of medical users
  • Near someone under the age of 21
  • Near an on-duty school bus driver, police officer, firefighter or corrections officer

Compassionate Use of Medical Cannabis Program

On August 12, 2019, changes to the Compassionate Use of Medical Cannabis Program became effective. The Compassionate Use of Medical Cannabis Program Act is a permanent program and the “pilot” designation has been removed.

The Illinois Department of Public Health is partnering with certified local health departments (LHD) to provide assistance to qualifying patients with completing their applications for the Medical Cannabis Registry Program.

Technical Assistance

The Illinois Department of Public Health (IDPH) is converting the existing Medical Cannabis Registry to a new system – Entellitrak. The conversion process will require the current system to go down beginning June 7, 2021.

Compassionate Use of Medical Cannabis Program
