Use these portals to access information about your care as a hospital patient, including emergency services, imaging, and laboratory services. These portals will connect you with your UP Medical Group provider (s).

At your request, our Patient Access staff or your health care provider will provide you a PIN number and authorize the system to send you an email with instructions on how to activate your account. When a message is sent from our office, you will receive an e-mail stating that you have a new message from Hunterdon Otolaryngology and Allergy Associates and are directed to log into the portal to review. Each family member who signs up for the portal will receive their own log-in and password. At this time, patients younger than 18 years old are excluded from the portal.
Our patient portal provides an additional opportunity for you to contact our office regarding non-urgent medical matters, routine follow up and administrative questions. If you have an urgent medical condition, call our office immediately or dial 911.

Established patients may also contact our patient portal helpline at 518-824-8620 Monday through Friday between 9 a.m. and 5 p.m. for assistance in registering for the Patient Portal.
With My Health Rec, you can: 1 Privately view some pathology, radiology and lab results online. 2 Access health records such as immunizations, medications and allergies 3 View hospital records such as discharge summaries and notes about procedures, progress and consultations (including wound care, PT/OT/SLP, dietary, etc.) 4 Download health records (history and physical) to easily share with doctors 5 Save time by paying health bills online
When it’s time for your appointment open the HHHN app (or log in from your computer) then tap or click on “Start Video Call”. Wait until your provider starts – don’t navigate off this screen. You’ll be able to see and talk with your provider (and nurse). When your visit is over, close the app or browser page.
Any active patient may be eligible to register for and use the Patient Portal. If you are authorized, a family access account can be created that will allow you to access selected family members’ health information.
