Here's how: 1. Go to your Patient Portal (links above). 2. Click 'I am new here.' 3. Click 'Accept.' 4. Enter the token you received from the health center or Patient Services. 5. Enter your email address. 6. Create a username and password. 8. Create a security question. 9. Access your Patient Portal. *Lost your token or never received one?
