How do I sign up for an appointment in the portal?

As of 10/27/21 this portal is now live! Any patient who provides a valid email address and schedules an appointment will automatically be signed up in this portal. Click here if you aren't scheduling any upcoming appointments but would like to sign up. Please provide the patient's name, date of birth and personal email address.
