Capital Health Medical Group offers two free online patient portals to give you access to your health information and information about any of your visits to our offices. One portal provides information for current visits. One portal provides information for visits prior to September 13, 2016.

How do I get help enrolling in the new hospital portal?

For assistance with the new hospital portal or to obtain your Community Medical Record Number which will be needed to enroll, please contact the Health Information Management Department at 609-303-4085 (Monday through Friday, 8 a.m. - 4:30 p.m.).
