Cornell Scott Receives Q-House Approval for 20 Years

Alders unanimously voted in support of an agreement that will see Cornell Scott Hill Health Center lease 15,374 square feet across two floors at the new, soon-to-open “Q House” community center at 197 Dixwell Ave. The federally qualified community health center will relocate from its current neighborhood location across the street in Dixwell Plaza.

What Is an Advocate?

At Cornell Scott-Hill Health Center, we advocate for both patients and workers in order to resolve complaints under our policies and procedures. Whether you need a PCP change completed, or would just like to file a complaint because you were unhappy with an experience, we are here to help.

Enrollment & Eligibility

Whether you are in need of the SNAP (Supplemental Nutrition Assistance Program), are not insured, are underinsured, or need help in any way, the Patient Advocacy & Enrollment Department is here for you. We have a team of certified specialists in different locations who can help guide you and connect you with other resources as well.

Insurance

We accept Medicare, Medicaid (Title 19), HUSKY, and most commercial insurance plans.

Sliding Fee Scale

If you are uninsured or underinsured, you may qualify for our sliding-fee discount program. Our sliding-fee discount program allows patients who qualify to pay a reduced fee for many of the Health Center’s services.
